Social Worker - HUD-VASH Program

About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →The incumbent is assigned to the VA Nebraska-Western Iowa Medical Center and provides services to Veterans treated in the HUD-VASH Program.
- →The social worker provides outreach, assessments, referrals and case management services to homeless veterans identified for possible participation and who are accepted to the HUD-VASH Program.
- →The incumbent is assigned to the Lincoln site of the VA Nebraska-Western Iowa Medical Center.
- →The social worker will be assigned to the Mental Health Service Line and will be supervised by the HUD-VASH Program Coordinator or Supervisor.
- →Major duties include, but are not limited to the following: The incumbent is responsible for providing independent case management for a caseload of veterans in the HUD-VASH program.
- →The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
- →The incumbent performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
- →The incumbent verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
- →The incumbent is responsible for ensuring that veterans experiencing homelessness are considered as primary participants for program acceptance.
- →The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.
- →The incumbent is responsible for providing HUD-VASH case management.
- →Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing.
- →The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH case manager.
- →The incumbent coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
