About the Role

This position will be primarily assigned to the Warriors Walk Hospice unit. The incumbent serves as a member of an interdisciplinary treatment team and participates in interdisciplinary treatment team meetings and completes the assigned social work sections of the Minimum Data Set (MDS). The incumbent works within the auspices of Geriatrics and Extended Care (GEC) Service provides social work coverage for those Veterans actively being treated by the Columbia VA Oncology Department.

What You'll Do

  • Total Rewards: Total Rewards of a Allied Health Professional Major Duties Include: The incumbent will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
  • The incumbent is responsible for developing the treatment plan and discharge plan (when appropriate) and setting achievable treatment goals with the Veteran/family.
  • The incumbent will attend treatment team meetings and complete all MDS entries and the Social Work Care Plan for team meetings in comprehensive and timely manner.
  • Throughout the course of treatment, the incumbent must serve as the subject matter expert on VA and/or community resources.
  • The incumbent will collaborate with other service providers in assessing and reassessing the Veteran's needs.
  • The incumbent is responsible for educating the Veteran, family and or significant others of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker.
  • The incumbent will refer the Veteran to needed services.
  • The incumbent is responsible for using the current social work resource file of VA and community social service programs and enhancing the content to the benefit of Veterans.
  • The incumbent understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the Veteran/family.
  • When appropriate and feasible, the incumbent will educate and encourage and Veteran/family to advocate on their own behalf, thus fostering a sense of independence and empowerment.
  • The incumbent will enter all Veteran/family contacts in the electronic record using appropriate formats and templates.
  • The incumbent will establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies and act as a consultant as needed.
  • The incumbent will participate in activities that obtain feedback from Hospice/Oncology departments and work to enhance services as needed.
  • Must perform all other duties as assigned.
