About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • Total Rewards of a Allied Health Professional The Social Worker in this position functions as the Facility Outreach Social Worker for VA's Health Care for Homeless Veterans Services.
  • The incumbent is responsible for outreach services to homeless veterans in communities and rural areas outside the main Medical Center or CBOC campuses.
  • Major duties and responsibilities include, but are not limited to: Responsible for interviewing veterans and their family members/significant others to establish facts about the veteran's homeless situation, presenting problems and their causes, and the impact of such problems on the veteran's functioning and health as part of a comprehensive psychosocial assessment.
  • Provides appropriate, comprehensive, high quality psychosocial assessments (to include HOMES form), evaluation of the client's situation, including the veteran's reaction and ability to deal with it, and arrives at a reasoned conclusion.
  • Develops psychosocial treatment plans in coordination with interdisciplinary team members, including goals for psychosocial clinical treatment and vocational rehabilitation or placement.
  • Provides clinical services to veterans and family members/significant others in support of the veteran's treatment.
  • Gives advice, guidance, emotional support, and other assistance.
  • Establishes a continuing relationship with the veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate.
  • Determines day to day work activities and decisions and is expected to make independent judgments and decisions in providing services to veterans, regardless of complexity and difficulty of the problem presented.
  • Meets the needs of customers while supporting VA missions.
  • Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner.
  • Provides the customer with consistent information according to established policies and procedures.
  • Handles conflict and problems in dealing with the customer constructively and appropriately.
  • Perform other duties as assigned.
