About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →The HCHV Case Manager assists veterans with accessing documents that will aid n accessing housing.
- →This may include social security, state identification, birth certificate or credit history.
- →May transport Veterans on housing searches or for documents to aid in securing housing.
- →Conducts psychosocial assessments; develops treatment plans in collaboration with the Veteran/family and with he interdisciplinary treatment team.
- →Possess the knowledge and skills to implement treatment modalities and provide counseling or psychotherapy for individuals, families and groups.
- →Possesses the skills to perform psychosocial interventions and measure outcome results.
- →Proficient in coordinating community-based services, including information and referrals for additional services from other government programs and community agency programs.
- →Identifies high risks patients and provides ongoing case management services.
- →Case management may continue from initial contact through housing placement to ensure continuity of care for the Veteran.
- →Provides crisis intervention services, seeking to address the cause as well as presenting complaints while engaging family and other support systems as appropriate.
- →HAs functional knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures, including acute, chronic and traumatic illnesses and /injuries, common medications and their effects, side effects, and medical terminology.
- →Independently provides consultation of services to other VA staff regarding the psychosocial problems on health care and adherence to treatment plans.
- →Has specific knowledge of Veteran benefits and services relating to special programs, service-connected pension.
- →Facilitates referrals based upon Veteran needs and eligibility.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
