About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →Total Rewards of a Allied Health Professional This position is for a Social Worker assigned to the West Texas VA Health Care System Patient Aligned Care Team.
- →This position uses a high levelof skill in assessing and treating the complicated psychosocial problems of veterans.
- →Social Work responsibilities include the assessment of psychosocial problems, often at a time when the Veteran is experiencing crisis and include the provision of short-term, solution-focused counseling with veterans and/or their family members/significant others.
- →Referrals typically originate from members of the interdisciplinary team, from Veterans and/or their family members/significant others coming as walk-ins to seek assistance, and from community professionals.
- →Duties include but are not limited to: Completes an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
- →Contributes to the development of the treatment plan and setting achievable treatment goals with the Veteran and family members/caregiver.
- →Uses the current Social Work resource file of VA and community social service programs and enhances the content to the benefit of veterans.
- →Understands the difficulty of bureaucracy and acts as an advocate when it serves the best interest of the Veteran and family members/caregiver Makes rapid assessments and develops crisis management plans.
- →Provides education related to VA and community resources, entitlements, Advance Directives/Living Will and will refer Veterans and family members/caregiver to the appropriate interdisciplinary team member for identified health education needs.
- →Reviews the progress notes from other providers to accurately determine the strengths and limitations of each Veteran.
- →Educates the Veteran, their family members/caregiver and the team of all the options available to them and will collaborate with the Veteran and family members/caregiver on the preferred option.
- →Responsible for the monitoring of, and discharge planning for, Veterans assigned to the PACT Team while Veterans are hospitalized in any number of local medical facilities.
- →This includes on- site review and assessment at local facilities when appropriate and necessary.
- →Provides the Veterans and their family members/caregiver with ongoing supportive counseling.
