About the Role
The Long Beach VA Health Care System is recruiting for (1) Social Worker (Community Resource and Referral Center) The incumbents primary responsibilities will focus upon Veterans experiencing or at risk of homelessness with the goal of connecting them to housing (e.g., permanent, transitional), health care, job development programs, prevention services and other VHA and non-VHA benefits.

- →Enrolls and provides case management services to Veterans as clinically indicated, with special emphasis on serving those who are the most vulnerable.
- →Arranges for provision of care through VHA or community resources in cases in which the Veteran is at risk and eligibility is not yet determined by VHA medical facility HEC Enrollment and Eligibility Staff or the Veteran is only eligible for limited services based on discharge status that is other than honorable.
- →Utilizes social work professional knowledge and experience as it relates to medical and mental health diagnoses, disabilities, and general treatment procedures.
- →Participates in collaborative planning with the Veteran, family, and the multi/interdisciplinary treatment team.
- →Provides a clinical assessment that supports the Veterans current situation or need based on services rendered in the encounter.
- →Obtains all required safety training, including training related to suicide risk reduction and working safely with high-risk Veterans, in both office and community settings, such as but not limited to those with severe mental illness and substance use disorder (SUD), those with a history of violence, and those with an involvement with the justice system.
- →Provides interventions in reducing psychosocial stressors to lessen suicide risk and assist with safety planning in support of VHA mission and goal to reduce suicide for our Veterans.
- →Work Schedule: Monday-Friday; 7:30am-4:00pm Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized EDRP Authorized: This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
- →You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
- →Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
