About the Role

The incumbent is responsible for the coordination, and provision of Social Work services to Veterans and their families in the Community Living Center (CLC). The Social Worker operates under the license of another licensed clinical social worker within an interdisciplinary treatment team in the CLC.

What You'll Do

  • Duties may include, but are not limited to: The Social Worker will develop an assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others.
  • The goal of the assessment is to highlight the Veteran's strengths, limitations, abilities, preferences, risk factors, internal/external supports, potential barriers to safe discharge planning, and service needs to optimize the Veteran's functional status.
  • The incumbent demonstrates an understanding of diagnoses and the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs, and age associated illnesses and conditions.
  • The social worker, working within the interdisciplinary treatment team, will participate in developing a mutually agreed upon treatment plan, to include discharge plan, with the Veteran and family (when possible), which includes a direct course of treatment.
  • Throughout the course of treatment, the social worker is a subject matter expert on VA and/or community resources.
  • The social worker is responsible for educating the Veterans and families on the available services and assisting them in establishing the appropriate referrals based on the Veteran preference.
  • The social worker understands the intricacies of bureaucracy and will act as an advocate when it serves the best interest of the Veteran/family.
  • The social worker is experienced in making rapid assessments and developing crisis management plans with the interdisciplinary team.
  • The social worker will participate in emergency response planning and training and may be involved in the response to all types of threat events including but not limited to natural disasters and internal and external threats of violence.
  • The social workers possesses knowledge and ability to implement Evidence Based treatment modalities, provide individual, group, or family psychotherapy, supportive contact and psychosocial education to Veterans and families.
  • The social worker documents independently in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up, and referral.
  • Work Schedule: Monday-Friday, 8:00AM-4:30PM Telework: May be available as determined by the agency/facility.

  • You must be a U.S.
