About the Role

The Caregiver Support Program Social Worker primary responsibilities are providing clinical services and interventions; program development; Caregiver, Veteran and staff education on Caregiver issues; community outreach; resource development; continuous quality improvement activities; and evaluation/consultation.

  • Home Visits may also be required for follow-up to identified high risk situations.
  • Conducts all required assessments with Veterans and caregivers on telephone and other forums which may include telehealth and VA Video Connect.
  • Participates in direct Caregiver and Veteran contacts to identify and assess Caregiver stress burden and the development and application of clinical treatment interventions.
  • Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
  • Assists the Caregiver Support Coordinators with program assigned duties.
  • Conducts Caregiver and family clinical interventions to facilitate the identification of Caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues.
  • Develops and facilitates Caregiver support groups (face-to-face, telephone, or other virtual methods) and educational programs.
  • Distributes educational materials provided by VA, local, state and national Caregiver organizations.
  • Assists with coordination of events and programs for the annual National Family Caregiver Month activities each November.
  • Inclusive of announcements, advertising of month long activities, and mobilization of participants, educational sessions, and Caregiver recognition events.
  • Demonstrates knowledge of community resources for Veterans and Caregivers.
  • The incumbent effectively communicates with and utilizes other disciplines (e.g., nursing, medicine) to facilitate treatment planning and implementation.
  • Incumbent effectively communicates with and utilizes community agencies to facilitate treatment, discharge planning, and continuity of care.
