About the Role

This position is located at Lebanon VA Medical Center within the Social Work Services service line. The Social Worker will be assigned to Acute Care Inpatient Medical Care units.

What You'll Do

  • The Social Worker is responsible to provide a variety of services for Veterans admitted to the hospital including psychosocial assessments, treatment planning, short-term, and solution focused counseling with individuals and families who may be in crisis, education and referral for resources and benefits, and case management.
  • Responsibilities will also include consultation and education to Medical Center staff and service providers, the collection and maintenance of statistical data, program planning evaluation and modification.
  • The Acute Care social worker in this Veteran-centric approach often functions by coordinating services and care with a panel of Veterans to ensure that health care meets the needs, as defined by the Veteran.
  • Social Workers are uniquely prepared for this role by virtue of their education, training, and experience which prepares them to understand the person-in-situation and the influences that family and community have in impacting the health-care needs of Veterans Major duties include, but are not limited to: Conducts psychosocial assessments to determine needs of veterans and family members.
  • Participates in the treatment planning process with other disciplines.
  • Participates in interdisciplinary team rounding, discharge planning, and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
  • Provides individual, family and group counseling as appropriate.
  • Acts as a consultant to other team/staff members.
  • Provides consultation/education to colleagues, other team members including medical residents on social work practice and VA policy and procedure.
  • Acts as liaison between VA and community resources, to include marketing and public relation duties as needed.
  • Provides case management to veterans and families, as needed, throughout the continuum of care.
  • Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
  • Coordinates treatment and communicate with the care lines to facilitate continuity of care for veterans.
  • Maintains data and statistical compilations to comply with JCAHO, VA and medical center policy and procedures.
