About the Role
The Senior Social Worker is assigned to the Social Work Service with various responsibilities to provide social work services to adult and geriatric Veterans receiving care, as well as provide open access to Veterans seeking these services and timely response to individuals in psychosocial crisis in settings and programs such as Patient Aligned Care Team in CBOCs, Emergency Department, Home Based Primary Care, and Integrative Chronic Pain Program.

- →The Senior Social Worker of the Emergency Department for the northern Arizona Healthcare System Social Work Services provides social work services in the Emergency Department (ED) and is assigned to provide support and assessment of psychosocial functioning and factors impacting Veteran overall heath and functioning.
- →The Emergency department Services Snr.
- →Senior Social Worker- (Integrative Chronic Pain Program) The Senior Social Worker is a member of the Integrative Chronic Pain Program team at NAVAHCS as an integral part of the treatment team providing care coordination and clinical counseling to Veterans within the program.
- →The Senior Social Worker works in the specialty medical area providing services to Veterans diagnosed with chronic pain.
- →Services include provision of high-quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the Veteran.
- →This is accomplished in collaboration with the other team members of the interdisciplinary treatment team.
- →Senior Social Worker (Home Based Primary Care) The Senior Social Worker (GS 12) is assigned to the Home Based Primary Care (HBPC) Program and serves as an advanced practice clinician responsible for delivering comprehensive social work services to adult and geriatric Veterans with complex medical, psychosocial, and functional needs.
- →This position requires independent clinical licensure and advanced skills in assessment, intervention, and program coordination.
- →The incumbent provides expert-level psychosocial assessment, treatment planning, and case management, and demonstrates leadership in program evaluation, quality improvement, and interdisciplinary collaboration.
- →The Senior Social Worker serves as a consultant to other staff, provides clinical supervision to trainees or less experienced social workers, and contributes to policy development and implementation.
- →Senior Social Worker (Patient Aligned Care Team- Community Based Outpatient Clinic) This Senior Social Worker is assigned to Patient Aligned Care Teams (PACT) in a CBOC with responsibility to provide social work services to adult and geriatric Veterans receiving care in at the Northern Arizona VA Health Care System.
- →This Senior Social Worker will provide open access to Veterans seeking these services and timely response to individuals in psychosocial crisis in PACT as well as supporting the behavioral health needs of those in crisis.
- →The incumbent is responsible to provide a variety of services for Veterans, including psychosocial assessments, treatment planning, short-term, solution focused counseling with individuals and families who may be in crisis, education and referral for resources and benefits, and case management.
Requirements
