About the Role
Incumbent serves as Senior Social Worker for the Renal/Hemodialysis Program and is administratively responsible for a clinic program and providing treatment to patients in a specialty area. The Senior Social Worker provides social work services at the Kansas City VA Medical Center (KCVAMC) to Veterans in need of inpatient and outpatient services in the nephrology/hemodialysis program. The Senior Social Worker also provides transplant assessments to various areas.
What You'll Do
- →VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional Provides both inpatient and outpatient social work services and supporting the interdisciplinary care team in setting up Renal/Hemodialysis services in the communities in which the Veteran resides and making recommendations for needs upon discharge.
- →Provides clinical case management to veterans receiving Renal services.
- →Provides transplant assessments in various areas as needed.
- →Addresses diminishing capacity, elder abuse including self-neglect, and probate conservatorship, and money management/fiduciary/ representative payee.
- →Educates patients and caregivers about available services and facilitate appropriate referrals based on the Veteran's needs and eligibility with the goal of maintaining/improving the patient's functional status and ability to function in the community.
- →Provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
