About the Role

The Senior Social Worker functions as part of the Patient Aligned Care Team (PACT) interprofessional primary care teams, and delivers brief, consultation-based mental health services to Veterans, primary care providers, and allied team members. These positions provide support to the National Center for Warrior Independence at the VA Greater Los Angeles Healthcare System in Los Angeles, California.

  • Duties may include but are not limited to: Mental Health - Assertive Community Treatment (ACT): Provides advanced-level intensive case management services to assigned Veterans consistent with the goals and methods of ACT and the VA Mental Health Initiative and with all relevant Medical Center policies and procedures.
  • Assist with collecting, evaluating, and monitoring Strategic Analytics for Improvement and Learning (SAIL) and Northeast Program Evaluation Center (NEPEC) Data.
  • Independently conducts advanced-level psychosocial assessments and psychosocial treatment to Veterans diagnosed with severe, persistent, and complex mental illness.
  • Social Worker ensures mobilization of environmental and organizational supports, including family members, professional, and other interested parties.
  • Participates in team staffing regarding patient selection, determination of treatment plans and goals, progress evaluation, discharge planning, and after care.
  • Works assertively and consistently with inpatient teams to assure continuity of care by providing consultation regarding clinical needs, treatment plans, and case management services for assigned patients.
  • Completes clinical documentation according to ACT, Medical Center, and Social Work standards; this includes but is not limited to progress notes, clinical assessments and progress reports, the Treatment Planning Review Form, and the Rehospitalization Review Form.
  • Works with other clinical programs and product lines that share in the care of assigned ACT patients, including but not limited to each product line and Rehabilitation Care Program.
  • Primary Care-Mental Health Integration (PCMHI): Assigned to work with special patient populations who present highly complex mental health problems in primary care and provide direct clinical services in this integrated program.
  • Serves as a mentor to other Social Workers, particularly those at the entry level.
  • Provides clinical supervision for unlicensed Social Workers, Social Work Associates and/or MSW Advanced Practice Social Work Interns.
  • Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services.
  • Demonstrates knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance use disorders, common medications and their effects/side effects, and medical terminology.
