About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The duties of the Domiciliary Care for Homeless Veterans (DCHV) Admissions/Transitions Coordinator and Senior Social Worker include: Program Coordination (75%) Managing admissions and transitions for Veterans into the DCHV program, collaborating with medical and psychiatric providers, community partners, and across VISN 15.
  • Coordinating assessments, screenings, and ensuring timely and appropriate placement.
  • Engaging Veterans and families in decision-making, providing referrals, and navigating alternative care options if needed.
  • Developing and implementing policies, measuring access and care continuity, and participating in quality improvement initiatives.
  • Collaborating with interdisciplinary teams and community resources to enhance service delivery and program accessibility.
  • Perform other related duties as assigned.
  • Clinical Practice (25%) Providing direct psychosocial assessments, evidence-based psychotherapy, and interventions tailored to Veterans recovering from homelessness, including those with co-occurring substance use and mental health disorders.
  • Participating in treatment and discharge planning, crisis management, and safety planning.
  • Mentoring and supervising staff and students, staying updated on best practices, VA policies, and community resources.
  • Educating Veterans and their families on available services, VA benefits, and community resources.
  • Documenting interventions and activities accurately in clinical records, ensuring continuity of care.
  • Perform other related duties as assigned.
  • These roles require advanced clinical judgment, strong collaboration skills, and expertise in mental health, substance use disorders, and homeless services.
  • Work Schedule: Monday - Friday, 8am - 4:30pm Compressed/Flexible: Not Available Telework: Ad-Hoc - This position is authorized for telework but has not been authorized as virtual or remote (i.e., must be able to work at the location the position is being announced for).
