About the Role
The Community Care Social Worker serves as the Geriatric and Extended Care (GEC)/Community Care Program GS-12 Social Worker. The GS-12 Senior Social Worker has experience that demonstrates advanced practice skills and judgement. Senior social workers must be licensed or certified by a state at the advanced practice level.
What You'll Do
- →VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Major duties include but are not limited to: The incumbent independently completes thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families.
- →The incumbent must be able to utilize the assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable.
- →The incumbent possesses a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses, common medications and their effects/side effects, and medical terminology.
- →The incumbent will serve as a liaison between Veterans, their families, residential care sponsors and VA and community resources in order to ensure thorough delivery of services.
- →The incumbent will have knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies.
- →The incumbent completes the Suicide Risk Assessment shared template in the Computerized Patient Record System (CPRS) on all patients with suicidal ideation and on any patient whose record is flagged "High Risk for Suicide." The incumbent will complete on-site inspections of CNH and CADHC as mandated.
- →The incumbent is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility.
- →Work Schedule: Monday - Friday; 0800 - 1630 Recruitment Incentive (Sign-on Bonus): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
- →Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Requirements
