This incumbent functions as a Senior Licensed Professional Mental Health Counselor (LPMHC) in the Mental Health Clinic's Behavioral Health Interdisciplinary Program (BHIP). The Senior LPMHC serves as the Team Lead for the BHIP and is responsible for overseeing the BHIP policies and procedures as well as ensuring Veterans timely access to care within the BHIP Team.
What You'll Do
- →Duties include, but are not limited to: Independently conducts psychosocial assessments.
- →Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team.
- →Possesses the skills to offer psychosocial interventions and measures outcomes to interventions.
- →Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
- →Participates in scheduling, triaging, and monitoring the flow of patients in the MHC and assisting with admissions of MHC patients to inpatient psychiatry unit when indicated.
- →Recognizes emergency situations, summons appropriate help and responds appropriately including provision of case management services, liaison to family members and crisis intervention services.
- →Establishes and maintains effective therapeutic relationships with veterans and their families. independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills.
- →Contributes to interdisciplinary care planning conferences, BHIP huddles and Team Meetings.
- →Possesses general knowledge of Veterans' benefits and services relating to special programs, service-connected compensation and non-service-connected pension.
- →Facilitates referrals based upon veterans needs and eligibility.
- →Provides concrete social work services such as resource allocation, referrals for food, housing, and /or transportation, and may assist Veteran clients with local, State and Federal benefits applications.
- →Assists Veterans with Advance Directives.
- →Provides consultation services to other staff and students regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
- →Develops and implements methods for measuring effectiveness of counseling practice and services in specialty areas, utilizing outcome evaluations to improve Performs other duties as assigned.
