About the Role
The Registered Nurse (RN) Women's Health Care Coordinator (WHCC) is responsible for providing competent, evidence-based care to assigned patients. The RN assesses, plans, implements, and evaluates care based on age-specific components, assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
What You'll Do
- →Start your VA career here Aurora Chamber of Commerce The Women's Health Care Coordinator (WHCC) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
- →Duties of the position are, but not limited to: Influence care outcomes by collaboration with members of the interdisciplinary team.
- →Knowledge and active participation in the unit/program level quality improvement process and initiatives as well as customer service programs.
- →Provide patient management, care coordination, and disposition planning for Veterans, including but not limited to addressing mammography, cervical cancer screening, and/or bone density testing.
- →Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
- →Coordinate care with multiple providers across all levels and sites of care.
- →Assure services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to Veterans.
- →Provide oversight of the patient's journey through the diagnostic and treatment continuum, ensure that all steps in the process are scheduled, timely and completed, while tracking the plan of care.
- →Contact Veterans, facilitate appointments and authorizations, track receipt of test results and other medical documentation, and notify providers of needed follow up care.
- →Interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards.
- →Anticipate and implement preventive patient care measures.
- →Provide guidance and education to patients and caregivers and collaborate with other professionals at all levels.
- →Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation with the interdisciplinary care team.
- →Ensure the plan of care and educational requirements are individualized and tailored specifically to support each individual patient's needs.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
