Registered Nurse- Spinal Cord Injury Home Care

About the Role
The VANJ Healthcare system is currently recruiting for one outpatient Registered Nurse to serve within our Spinal Cord Injury/ Disorder (SCI/D) Service Home Care (HC) Program. The SCI/D Home Care Program is a special PACT program designed for Veterans living with SCI/D, Multiple Sclerosis, and ALS which impact their health and functioning on a daily basis. Ongoing visits are made, in person or by video to home technology as clinically appropriate.
What You'll Do
- →Duties include, but are not limited to: Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care.
- →Provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the SCI-D, MS and ALS Homecare Program.
- →Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
- →Plan and deliver technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
- →Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
- →Makes home visits to Veterans in the program.
- →Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
