What You'll Do
- →The Patient Aligned Care Team (PACT) Nurse Care Coordinator is a self-motivated professional nurse who provides leadership in both direct and indirect patient management.
- →This includes coordinating care with physicians, nurse practitioners, medical residents and students, allied health professionals, and hospital systems.
- →The role encompasses all managed care nursing settings and reports directly to the Nurse Manager of Managed Care at the site.
- →The RN is responsible for organizing and overseeing care for patients assigned to the PACT, focusing especially on those at higher risk, whether they are admitted as inpatients and transitioning to outpatient care, or those remaining in the outpatient setting.
- →The Nurse Care Coordinator demonstrates leadership, clinical expertise, and creativity in navigating complex patient needs and care plans.
- →Responsibilities include coordinating care across VA and non-VA healthcare systems, ensuring continuity and quality of care, and utilizing innovative care management strategies.
- →The RN empowers patients in self-management, prevention, and wellness-aligning interventions with each patient's individual goals and using motivational interviewing techniques to facilitate goal setting and achievement.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
