About the Role

The incumbent must analyze, evaluate, and advise on the effectiveness of work methods and procedures, departmental organization, manpower utilization, information and documentation systems, and/or similar functions of management that have an impact on Primary Care. The incumbent achieves this through the utilization of various systems, including the PCMM application and other primary care systems, labor mapping updates in DSS, and other internal and external systems.

What You'll Do

  • The incumbent is ultimately responsible for the correctness and quality of the information placed in Primary Care systems.
  • Specifically, the absolute direct full-time equivalent (FTE} clinical time associated in PACT for each clinical member of a PACT team in the facility and its substations must be accurate.
  • The incumbent will be responsible for the integrity of the Medical Center's primary care applications, its functionality, utilization, and resulting data.
  • The data validated and updated by the incumbent is the primary source data used for PACT model capacity, calculations, analysis, and resource needs.
  • The incumbent is the authorized facility point of contact and official trained to update primary care systems.
  • The incumbent validates data by determining the direct clinical FTE time.
  • The incumbent uses the PACT model capacity formula and adjusts panel size based on the direct clinical FTE in PACT, clinical and administrative support staff FTE (referred as teamlets) assigned to each PACT, rooms available to each PACT, intensity rate of each PACT panel, and number of Women Veterans assigned to each PACT panel.
  • The incumbent ensures the primary care systems are updated monthly with the exact room number(s) used by each and every clinical member of a PACT team at all locations.
  • Errors can result in facilities purchasing too many PACT teams (costing multi-million dollars of too many PACT teams) or not enough PACT teams (costing poor access and patient-centered care delivery).
  • Combined with the actions described in the previous paragraph, errors can result in variance of PACT model capacity panel sizes in any direction.
  • The incumbent establishes and defines local processes on patient attrition and assignment.
  • The incumbent then monitors PACT team composition, and updates the patient population within each PACT team to account for attrition (removing patients from PACT teams due to death, transfer, or inactivity) and reassignment (based on patient request, gender, morbidity, location, or other population).
  • The incumbent calculates, adjusts, and monitors panel sizes, PACT staffing (direct clinical FTE), and space (rooms assigned) as described above which impact PACT procedures, processes, and systems and other decisions.
  • The incumbent ensures that PACT teams have panel sizes that are appropriate to allow for adequate access for new and established patients, and are able to provide the best patient centered care possible, in collaboration with the Associate Chief of Staff for Ambulatory Care.
