About the Role
The Cincinnati VHA Medical Center is seeking a Primary Care PACT RN (Patient Aligned Care Team) at the Hamilton CBOC Clinic. This RN is responsible for providing competent, evidence-based sex-specific care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. - →This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
- →Major Duties may include but are not limited too: Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
- →Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
- →Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization.
- →Integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations.
- →Validates understanding of the rationale for care processes and decisions.
- →Questions clinical practices for the purpose of providing evidence-based care.
- →Demonstrates the ability to cope with and manage competing priorities.
- →Demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills.
- →All other duties as assigned.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
