About the Role

This LPN position is located in the Access & Office of Community Care (AOCC) section at the Birmingham VA Medical Center. The incumbent serves as an advocate within the community care program, is involved in continuous quality improvements, provides care and support for veterans using knowledge and skills to perform increasingly complex nursing functions within the community care continuum. Occasional travel of up to 25% is required for this position.

  • This is an advanced level LPN Position.
  • Duties include but are not limited to: Screens new consult for community care eligibility, specialty service requested, duplication and appropriateness of referring patients to community provider.
  • Process community care referral to completion and perform timely follow up as indicated.
  • Knowledge and skills of community care policies and procedures and referral pathways for community resources.
  • Collaborates, communicates, sets priorities, and organize assigned work to meet deadlines, ensuring compliance with established processes, policies and regulations.
  • Provide care support which will include activities of daily living, maintain a safe, clean therapeutic environment for patients while following all safety policies and procedures, assists with planning and participate in health care plan, and considers the needs of the individual patient for care including medication and equipment-assisted care.
  • Provides direct nursing care to adult and geriatric patients with complex problems according to nursing standards and Medical Center policy and procedure.
  • This includes, but is not limited to: Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and /or intravenous) as directed without error that may cause harm to patients. .
  • Demonstrates advance knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

  • As a condition of employment for accepting this position, you will be required to serve a 1 or 2-year trial period during which we will evaluate your fitness and whether your continued employment advances the public interest.

Benefits

  • This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
  • You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
  • Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
