About the Role
The Licensed Practical Nurse (LPN) is responsible for the delivery of care to veterans in the Specialty Care Clinics. The LPN is responsible for the treatment, observation, and care of the Veteran within the scope of practice to assist in the overall clinic patient flow, procedures, the check in and checkout process, scrubbing schedules, and running daily clinic with the Provider and the RN . The LPN provides direct and indirect nursing care to patients and families within the clinic.

- →The LPN is a key member of the Specialty Care and multidisciplinary team and is responsible for collaboration with other team members, promoting effective communication and ethical practice while integrating population health to meet the needs of the complex Veteran.
- →The LPN uses a systematic problem-solving process in the care of patients with chronic health care needs to provide individualized, goal-directed nursing care.
- →The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family.

- →Demonstrates ability to serve as a responsible member of the nursing team and interacts in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice.
- →Expected to exercise independent technical judgments in decision making without readily available supervision, and to possess and apply a comprehensive working knowledge of the principles, art and science of practical nursing.
- →The incumbent assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management, and supporting factors that impact customer satisfaction.
- →Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or others related to support duties.
- →Performs other related duties as needed or assigned to support the functions of the service as assigned by the clinic RN or Nurse Manager.
- →Observes, screens, records, and reports patient's response or changes in patient's condition.
- →Knowledgeable and capable of MyhealtheVet and portals to expand patient communication and education.
- →Flexible in adapting to work demands and numerous assignments.
- →Recognizes urgent or emergent patient care situations, seeks assistance of the RN and/or provider, and initiates appropriate emergency interventions as directed.
- →Provides effective education to patients and/or family member in relation to disease processes, medication and/or prescribed treatment regimens in collaboration with the interprofessional team.
