About the Role
About the Position: This position is located in the Care Management Services at Madigan Army Medical Center in Tacoma, Washington. This position also supports Pediatrics Case Management. Typical duty hours are Monday thru Friday from 0730 to 1600, with potential for alternative work schedule. This is a Direct Hire Solicitation.
What You'll Do
- →Provide practical nursing care to patients in the outpatient clinic with a variety of medical conditions, performing tasks such as ordering labs, radiology orders and referral results, and making future appointments.
- →Initiate and maintains medical histories on patients, records observations, and identifies symptoms for use by the clinician.
- →Conduct focused nursing assessments and contributes to the care plan in collaboration with the healthcare team.
- →Educate patients and families about prescribed treatments, reinforcing adherence to the medical plan.
- →Coordinate care across specialty care settings and in the community.
- →Document care in the electronic health record.
- →Collaborate with the healthcare team to develop and adjust care plans based on patient progress and feedback.
