About the Role
St. Cloud VA Health Care System is seeking a Primary Care Physician to care for Veterans in St. Cloud, MN. This position is eligible for the Education Debt Reduction Program (EDRP). An incentive is authorized for highly qualified applicants.

- →The Primary Care Provider is an integral member of the Primary Care Service Line who may be assigned to GAP coverage or Patient Aligned Care Team (PACT teamlet) based on clinical need and coverage.
- →Primary care physicians will participate in daily huddles with other teamlet staff and primary care systems redesign projects.
- →Duties include chronic disease management, preventative health care and evaluation of episodic acute illness to a predominately male population.
- →FUNCTIONS OR SCOPE OF ASSIGNED DUTIES: Duties as a PACT Provider: Participates in daily huddles with other teamlet staff and primary care systems redesign projects.
- →Manages a panel of patients under Primary Care Management Model (PCMM) guidelines; recommended panel size at the division level is calculated based on patient complexity, support staff and room availability.
- →Each provider's panel size is specific to their practice and setting.
- →Provides clinical care for the patients on their panel.
- →This includes management of acute and chronic conditions, medication management, preventative care and management of clinical reminders, timely management of clinical alerts and home care orders, and other needs of the patient.
- →A primary care provider is expected to assist with coverage of other department member panels as needed do to absences or based on workload.
Requirements
- ✓Citizenship: non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓English Language Proficiency.
- ✓In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued dentification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
