About the Role
The Long Beach VA Health Care System is currently recruiting one (1) Outpatient Clinical Pharmacy Technician within Pharmacy Service. The incumbent works autonomously using independent judgement to perform a full range of duties to support Outpatient Pharmacy Services specifically Outpatient Clinical Pharmacy programs at least 25% of the time under the general guidance of lead clinical technicians and supervision of clinical pharmacists and Pharmacy Service supervisor(s).
What You'll Do

- →Additionally, identifies health related conditions while the patient is being evaluated.
- →Including, but not limited to: new patient medication intake reviews and polypharmacy screening.
- →Routinely reviews the computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies/adverse drug reactions.
- →Reports identified problem patients to the assigned clinical pharmacy specialist for evaluation.
- →Routinely reviews laboratory monitoring reports for selected disease states and identifies patients with values outside of the specified parameters.
- →Reports identified patients to the assigned clinical pharmacy specialist for evaluation.
- →Inputs non-controlled new and renewal prescriptions into the computer system on behalf of the provider (subject to provider's authorization signature).
- →Prescriptions entered by the clinical technician must be signed by a provider prior to pharmacy review and verification.
- →Serves as a subject matter expert in the day-to-day advising pharmacists and technicians along with problem solving in assigned areas (e.g.
- →PACT, Pain Management, Anticoagulation).
- →Assists in collecting and analyzing data related to performance improvement projects, workload and clinic outcomes.
- →Identifies, problem solves and implements corrective actions to maximize outcomes in assigned clinical areas.
- →Verifies patient identification (e.g., double patient identifiers).
