About the Role
The PACT Outpatient Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.
What You'll Do
- →Duties include but are not limited to: Determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
- →Provides gender-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
- →Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
- →Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
- →Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- →Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum, including the Veteran's goals of care.
- →Evaluates practice in an ongoing process, based on best evidence.
- →Provides peers with formal or informal constructive feedback for improvement.
- →Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care.
- →Fosters a safe and supportive environment conducive to the professional development of healthcare professionals.
- →Responsible for the documented PACT outcomes at the program or service level.
- →Performs other duties as assigned.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
