About the Role
This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →This position is assigned at Hampton VA Medical Center - Western Branch Community Based Outpatient Clinic (WB CBOC).
- →The Outpatient Clinical Pharmacist supports the general operations of pharmacy including prescription/order processing, outpatient dispensing, patient consultation, and perform all pharmacy related activities as assigned and on any shift.
- →Duties include but are not limited to: Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
- →Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and/or in writing.
- →Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients, and caregivers.
- →Recommends formulary alternatives and completes non-formulary and restricted medication consult as assigned.
- →Manages various actions, as appropriate, for medication orders to include flagging prescriptions, managing view alerts, and generating unassigned discontinued orders using service reject.
- →Interpret various VA, Federal (FDA, DEA), other regulatory agency compliance standards (e.g., Joint Commission) Facility and Pharmacy Service policies and be able to apply the same to given situations.
- →Ability to exercise sound judgment in a variety of situations and be able to refer matters of policy making to a supervisor.
- →Supervises and directs pharmacy support personnel in the absence of a supervisor.
- →This includes verification of the daily activities of pharmacy technicians.
- →Works independently with minimal supervision.
- →Organizes and prioritizes work assignments.
- →Ensures pharmacy services are provided in a timely and efficient manner.
