The purpose of this position is to help plan, coordinate, implement, and evaluate community-based nutrition care, education, and promotion of services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Provide consultation and technical assistance to the Clinical Diabetes Program, Health Promotion Department, and other professional staff to help coordinate service unit wellness programs and other diabetes prevention programs.
- →Help review and assess program progress and operations to identify and make recommendations for improving the quality and assess the appropriateness of community nutrition services and programs.
- →Conduct nutritional assessment of clients, developing nutritional care plans with them and their families, consistent with cultural, social, and physiological requirements.
- →Record information in medical records (progress notes) regarding nutritional requirements, patient progress, education, follow-up, etc.
- →Report and summarize progress and activities at regular intervals and prepare periodic and special reports or projects.
Requirements
