About the Role

The incumbent is located at one of the sites of care for the Roseburg VA Health Care System (RVAHCS), including the main Roseburg campus and Eugene VA HCC. Travel may also be required within the Health Care System. The incumbent assists and cares for Veterans in outpatient specialty care setting, communicates with other members of the healthcare team regarding patient concerns and personal observations.

What You'll Do

  • Functions or Scope of Assigned Duties: Ability to assist in the full range of nursing care to patients/Veterans with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse.
  • Ability to communicate orally with patients/Veterans, families, interdisciplinary team and other personnel.
  • This includes serving as a preceptor to new NAs by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting.
  • Ability to recognize and react to emergent patient/Veteran care situations and intervene while waiting for assistance.
  • For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis.
  • Utilizes Point of Care (POC) glucose device to receive glucose readings for Veterans, refers deviation from normal range to appropriate health care staff Provides direct care for complex Veterans Observes subtle changes in Veteran's condition, (i.e. behavior shifts, mood, skin coloring, appetite) and acts within the NA's assigned role.
  • Documents care given, observations and responses on appropriate tools or forms.
  • Responds appropriately to subtle patient changes by contacting the appropriate nursing personnel.
  • Performs nursing procedures within scope of practice and according to policies for procedures (but not limited to): enemas, catheterization, colostomy care, skin care simple dressing, application of braces and prosthetic devices Works in small group activities/group appointments with Veterans according to the treatment plan.
  • Recognizes the Veterans' autonomy and promotes independence by means of respect, limit setting and individualized treatment.
  • Actively participates in Team and Clinic Huddles Utilizes equipment to promote Veteran self-care, including lift equipment Verbally provides patient information to the treatment team for inclusion in the Veterans' electronic health record, participates in team meetings for Veteran care planning.
  • Enters information about the Veterans in the chart, utilizing appropriate formats.
  • Participates in maintaining a clean, safe environment for the patient/Veteran There is no regular driving or travel associated with this position.
  • The selectee will primarily work in Eugene, but may occasionally be asked to assist in Roseburg if additional support is needed.
