About the Role

The Women's Health Patient Aligned Care Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.

What You'll Do

  • CTVHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care.
  • This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care.
  • This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being.
  • The Women's Health Patient Aligned Care Nurse duties and responsibilities include but are not limited to the following: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex gender-specific primary care.
  • Provides direct gender-specific Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.
  • Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels.
  • Provides gender-specific health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
  • Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
  • Demonstrates performance and leadership that is broad enough to improve gender-specific care for the Primary Care Department and facility.
  • Provides peers with formal or informal constructive feedback for improvement.
  • Supports colleagues and other nurses through gender specific primary care knowledge sharing to provide safe, quality nursing care.
  • Fosters a safe and supportive environment conducive to the professional development of health care professionals.
  • Evaluates outpatient outcomes of evidence-based decisions and practice changes for gender-specific primary care individuals, groups, and populations.
  • Responsible for the documented PACT outcomes at the program or service level.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • You may be required to serve a probationary period.

  • The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
