About the Role
The Nurse provides direct Veteran care and care coordination, as it pertains to Veterans and their care within the Home Based Primary Care Program (HBPC).
What You'll Do
- →The mission of the HBPC is to provide comprehensive, interdisciplinary, primary care in the homes of Veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective.
- →The HBPC target population is frail and/or chronically ill Veterans who require interdisciplinary health care teams, continuity and coordination of care, and the integration of diverse services to cover their complex medical, social, rehabilitative, and behavioral care needs.
- →These Veterans need comprehensive, longitudinal home care services as they age to maximize function, minimize institutionalization, and maintain quality of life.
- →The HBPC RN Case Manager will: Provide initial and continued assessment of nursing needs of the Veteran.
- →Teach the Veteran and/or Caregiver effective and efficient ways to manage health and care in the home.
- →Monitor the Veteran's condition and the care provided in the home.
- →Oversee the provision of direct care.
- →May delegate tasks to the HBPC LPN depending on the patient's medical needs.
- →Visit the Veterans monthly for the first 90 days the Veteran is enrolled in the HBPC program, then will visit at least every 60 days if supplemented by LPN visits so that nursing visits are provided on a minimum of once a month; will visit monthly if no supplemental LPN visits.
- →Complete the appropriate clinical reminders/screening.
- →Monitor the Veteran's physical and psychosocial status.
- →Notify the Primary Care Provider of changes in condition or care needs.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical evaluation/examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
