What You'll Do
- →The Military to Veterans Affairs (M2VA) Registered Nurse (RN) provides and leads the delivery of competent, evidence-based, patient-centered, and recovery-oriented nursing care within the M2VA outpatient setting.
- →The (M2VA) RN is responsible for providing competent, evidence-based care to assigned patients.
- →Major duties include, but are not limited to: Provides case management of care activities for post 9/11 service members and Veterans treated at the facility.
- →Access, plans, implements, and evaluates care based on age-specific components.
- →Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Follows procedures per established policies and guidelines.
- →Influences care outcomes by collaborating with members of the interdisciplinary team.
- →The nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
- →Provides clinical case management for severely injured, those with mental illness and/or ill post 9/11 service members/Veterans with identified case management needs.
- →The M2VA Case Manager RN begins the case management process prior to enrollment to the facility and works collaboratively with staff from other Veterans Affairs facilities, Military Treatment Facilities, and referring facilities.
- →Completes an in-depth assessment of functional status, acuity level, prognosis, and assesses the need for treatment service and resources.
- →Initiates contact with the patient and/or family to assist and ensures the transition to the VA is safe and continuous.
- →Monitors the patient status while the patient is receiving care at the facility.
- →Documents appropriately in the electronic health record, Care Management Tracking and Referral Application (CMTRA), and the Interagency Comprehensive Plan (ICP).

- ✓This period is considered to be an extension of the interview process and will provide your supervisor with an opportunity to evaluate your conduct and performance on the job.
- ✓During this period if it is determined that your performance or conduct fails to demonstrate your qualifications for employment, you may be terminated.
- ✓This position is in the Excepted Service and does not confer competitive status.
