About the Position: This position is located at Naval Health Clinic Lemoore, Lemoore, CA. The A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. This is a Direct Hire Solicitation
What You'll Do
- →Perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions.
- →Facilitate continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD), and anxiety disorders in adult beneficiaries.
- →Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools.
- →Recognize and report abnormal test results, changes in symptoms, and the need for follow-up.
- →Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.
- →Assist clinic with patient and staff education and provide individual and small group training in order to increase referrals and promote understanding of the role of the mental health care facilitator.
- →Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
