About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount up to $200,000 & eligibility period of one to five years are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply

What You'll Do

  • Major Duties: Coordinates, evaluates, and manages referrals, assessments, and reassessments for Veterans needing home care services through community agencies, using professional nursing knowledge and licensure.
  • Coordinates service delivery for increasingly complex patients enrolled in H&CBC programs such as Homemaker/Home Health Aide (H/HHA), Contract Adult Day Health (CADHC), Home Respite, Home Hospice, Purchased Home Skilled Care, and PACE.
  • Monitors home care program quality and operations using tools such as spreadsheets and relational databases to track various aspects of the H&CBC programs.
  • Conducts regular community program visits at prescribed frequencies to ensure safe, therapeutic care is provided and that all visits are appropriately documented.
  • Identifies and reports substandard or unsafe situations, takes appropriate corrective action with the community agency, and follows through until issues are resolved.
  • Communicates monitoring results and clinical findings to the Medical Director, Program Director, and/or interdisciplinary H&CBC team, and actively participates or leads interdisciplinary team meetings and patient care conferences.
  • Assesses patient and family learning needs and limitations, then plans, provides, and documents education and appropriate referrals to support effective home and community-based care.
  • Provides cross coverage for other H&CBC programs within Extended Care Service as assigned by program leadership, ensuring continuity of care across programs.
  • Leads or participates in performance improvement activities that enhance outcomes for the community programs and the overall H&CBC Program, including established PI efforts with community services and Extended Care Service.
  • Serves as the nurse expert on VA inspection teams for ADHC, H/HHA, Home Respite, Home Hospice, and CNH facilities, documenting inspection findings, recommending and following up on corrective actions, and maintaining monitoring reports for each agency.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
