About the Role
About the Position: This position is located at the 20th Medical Group, Shaw Air Force Base, South Carolina. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Provide comprehensive Disease Management (DM) Services.
- →Develop treatment programs to provide advanced level, specialized, and comprehensive professional nursing care.
- →Perform clinical or diagnostic procedures.
- →Provide nursing support to clinic functions during base operational readiness exercises/inspections or major events requiring healthcare assets.
- →Collaborate with healthcare professionals/departments to identify members needing additional education or clinical management of selected diseases.
- →Generate data through the use of assessments, screening, and other data tools to identify individuals with specific disease states/conditions/or risk.
- →Orient healthcare teams to the purpose of disease management activities, population health, clinical preventive services, and medical management.
