About the Role
This position is located at a Service Unit within the Indian Health Service (IHS). The primary purpose of this position is to provide professional community/public health nursing services through direct care to individuals, families, and identified populations through direct care, home and school visiting, and population-based assessment, prevention, and intervention activities. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Assesses the health needs of individuals, families, and the community served, taking into account the total situation in the family, the home, and in the community environment.
- →Develop individualized and family-centered care plans.
- →Implements and evaluates plans of care to support prevention, early intervention, treatment adherence, and continuity of care.
- →Shares health information and collaborates with patients, families and other community resources.
- →Educates and coaches people to make appropriate and informed decisions about health issues.
- →Apply population health assessment and surveillance findings to support targeted prevention, early intervention, and public health nursing activities within the assigned service unit.

Requirements
