About the Role
The Community Care (CC) Registered Nurse (RN, Provider Liaison serves as a clinical resource and care coordination specialist, ensuring timely, safe, and patient-centered services delivered through community providers. The Provider Liaison supports seamless transitions of care by facilitating communication among VA clinical teams, community partners, Veterans, and Third Party Administrators, while upholding VHA standards.
What You'll Do
- →Duties included but are not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising patient care.
- →Core responsibilities include clinical review, care coordination, provider liaison functions, and quality oversight.
- →Responsible for all elements of the nursing process and plays a key role in delivering high-quality, timely community care.
- →The Provider Liaison is responsible for executing a streamlined process for directing timely, coordinated care for Veterans.
- →The purpose of this process is to strengthen collaborative relationships with community-based providers by promoting clear communication, shared expectations, and timely follow-up on mutual clients.
- →The Provider Liaison demonstrates proficient clinical judgment, deliberate planning, and strong critical-thinking skills.
- →The Provider Liaison maintains comprehensive knowledge of internal referral workflows, clinical review criteria, utilization management standards, documentation requirements, community care regulations, and Veterans Health Administration (VHA) guidelines.
- →Using nursing expertise and systems-based practice, the Provider Liaison assesses, organizes, and facilitates care across the full range of available options, including internal VA services and community care.
- →The role includes serving as a liaison between VA and community providers to ensure Veterans receive timely access to appropriate services and to support the medical center's mission of high-quality care delivery.
- →The Provider Liaison complies with VA-mandated processes and uses health information systems to assess, document, exchange, and track care coordination activities.
- →Introduce our services and points of contact through brief outreach (email, call, or in-person visit).
- →Schedule periodic (e.g., quarterly or semiannual) check-ins with key community partners to review what is working well, address barriers, and identify opportunities to collaborate on future activities.
- →Acknowledge inquiries from community providers promptly.
- →Telework: Available.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
