About the Role
About the Position: This position is located at Army Health Clinic Munson, Fort Leavenworth, Kansas. This is a Direct Hire Solicitation A recruitment incentive may be authorized. Non-Federal work experience may be creditable for leave purposes. Salary negotiation may be available for those candidates who are new to Federal service.
What You'll Do
- →Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.
- →Analyze complex medical, psychosocial, and administrative problems to recommend effective solutions and improve outcomes.
- →Provide comprehensive assessments of patient health needs and develop individualized, cost-effective care plans.
- →Evaluate patient progress through follow-up assessments, documenting interventions and guiding informed decision-making.
- →Collaborate with multidisciplinary teams and serve as patient advocate to minimize barriers and optimize quality of life.
