About the Role
About the Position: This position is located at Evans Army Community Hospital at Fort Carson, Colorado. This is a Direct Hire Solicitation. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized.
What You'll Do
- →Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.
- →Analyze complex medical, psycho-social, and administrative problems to recommend effective solutions and improve outcomes.
- →Provide comprehensive assessments of patient health needs and develop individualized, cost-effective care plans.
- →Evaluate patient progress through follow-up assessments, documenting interventions and guiding informed decision-making.
- →Collaborate with multidisciplinary teams and serve as patient advocate to minimize barriers and optimize quality of life.
