About the Position: This position is located at Madigan Army Medical Center Joint Base Lewis McChord, Care Management Services, working as a Case Manager in the Madigan Internal Medicine Clinic The work involves collaborating with interdisciplinary health care team and the patient/caregiver to address areas of concern, guide informed decision making, and encourage the patient on quality-of-life improvement. This is a Direct Hire Solicitation
What You'll Do
- →Will serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).
- →Conduct comprehensive assessments of beneficiaries’ health status, functional limitations, and support systems to develop individualized plans of care.
- →Collaborate with providers and support staff to address these variances and recommend adjustments that improve care efficiency and patient outcomes.
- →Gather data from multiple healthcare agencies, document patterns affecting quality of care, and escalate concerns that may indicate system-level issues.
Requirements

- ✓Foreign Degree: A degree from a Foreign Nursing School.
- ✓Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
