About the Position: This position is located at Sheppard AFB, Texas. Salary negotiation may be available for those candidates who are new to Federal Service. Non-Federal work experience may be creditable for leave purposes. This is a Direct Hire Solicitation
What You'll Do
- →Conduct clinical assessments of patients health needs to formulate individualized, patient-centered care plans.
- →Educate patients, their families, and the healthcare team on treatment options, insurance benefits, and available community resources.
- →Arrange necessary health services, specialized referrals, and follow-up appointments across both military and civilian sectors.
- →Analyze complex cases to identify barriers or systemic problems related to healthcare access and utilization.
- →Review medical referrals for clinical accuracy and assist in routing patients to the appropriate specialty providers.
