About the Position: This position is located Family Medicine Service, Lacey Washington. What You'll Do
- →Serve as an Ambulatory Nurse performing independent, comprehensive telephone and clinical triaging and facilitate care coordination through a multidisciplinary approach.
- →Screen walk-in patients to assess the severity of the patient's condition and the need for provider intervention.
- →Review patient charts to assess the medical history and determine health care needs Recognize and report abnormal test results, changes in symptoms and the need for follow-up.
- →Instruct and demonstrate to patients the proper techniques for performing self-care activities.
- →Provide education and guidance to support patient self-administration of prescribed medications.
