About the Role
About the Position: This position serves as a Nurse (Clinical/Ambulatory) performing independent and comprehensive triage for patients, facilitate coordination with multi-disciplinary teams to provide the highest level of care, and provide education to patients. It is located at Redstone Arsenal, Huntsville Alabama in the Outpatient Clinic. This is a Direct Hire Solicitation
What You'll Do
- →Perform independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach.
- →Act as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention.
- →Screen patient charts to assess the medical history and determine health care needs.
- →Assist clinic with patient and staff education.
- →Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.
- →Develop appropriate nursing plans.
- →Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
