About the Position: This position is located at Army Community Hospital Weed at Fort Irwin, CA. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Non-Federal work experience may be creditable for leave purposes.
What You'll Do
- →Perform independent, comprehensive telephone and clinical triaging, and facilitate care coordination through a multidisciplinary approach.
- →Provide patient education as an integral part of the services offered.
- →Assist clinic with patient and staff education.
- →Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.
