About the Role
This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Tsaile, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Coordinates comprehensive case management and care for high-risk and at-risk patients, including assessment, individualized care planning, referrals, follow-up, and evaluation of patient outcomes.
- →Provides skilled direct nursing care across age groups, including triage, emergency treatment, clinical assessment, nursing interventions, and maintenance of accurate health records.
- →Coordinates multidisciplinary services and continuity of care by collaborating with patients, families, providers, specialty services, outside facilities, transportation resources, and other healthcare agencies.
- →Provides patient and family education, counseling, and preventive health support using culturally appropriate approaches to promote self-care, disease prevention, treatment adherence, and improved health outcomes.
- →Supports quality improvement, patient safety, and administrative/community outreach activities through documentation review, performance improvement efforts, staff and community education, and collaboration with the multidisciplinary healthcare team.
