About the Role
This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Chinle, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Coordinates case management and care coordination services for high-risk and at-risk patients, including referrals, follow-up care, chronic disease tracking, and multidisciplinary treatment planning.
- →Provides direct nursing care, patient assessments, triage, emergency interventions, and specialized clinical support for chronic, emergent, and complex patient conditions.
- →Monitors patient progress through disease registries, laboratory and imaging follow-up, medication reconciliation, discharge planning, and continuity-of-care activities.
- →Educates and counsels patients, families, and community members on disease prevention, treatment plans, self-care strategies, and available health resources using culturally appropriate approaches.
- →Collaborates with multidisciplinary healthcare teams, Purchased/Referred Care, community agencies, and transportation services to improve patient outcomes and support quality improvement initiatives.
