About the Role
This position is located within the Department of Nursing Services at Kayenta Service Unit in Kayenta, AZ. The position provides expertise in assessing, planning, implementing, and evaluating care and education for individuals with or at risk for Type 2 diabetes. It also provides staff training and clinical consultation to support coordinated, high-quality, cost-effective diabetes care and improved patient self-management. A REAL ID will be required, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Assess, plan, implement, and evaluate care for patients with or at risk for diabetes, following IHS Diabetes Standards of Care and coordinating treatment plans with the healthcare team.
- →Educate patients, families, and groups on diabetes pathophysiology, medications, nutrition, exercise, blood glucose monitoring, and prevention and management of acute and chronic complications.
- →Collaborate with providers and specialty departments (e.g., Optometry, Dental, and other clinics) to ensure patients receive recommended screenings, follow-up care, and services to prevent diabetes-related complications.
- →Monitor patient registries, conduct follow-up for overdue or high-risk patients, develop case management guidelines, evaluate patient outcomes, and participate in performance improvement, audits, and policy development.
- →Develop and implement diabetes prevention initiatives, provide in-service education and consultation to healthcare staff, and create culturally appropriate educational materials for patients and providers.
