About the Role
This position is located in the Kayenta Service Unit's, Division of Nursing, Outpatient/Pediatric Department in Kayenta, AZ. The role provides case management and clinical coordination for patients and responsibilities include patient assessment, care coordination, service integration, follow-up, and evaluation to improve continuity of care, patient flow, and overall clinic services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Provides outpatient nursing care and patient assessments for individuals across all age groups, including triage, treatments, medication administration, monitoring, and response to changing patient conditions.
- →Coordinates case management and continuity of care by arranging referrals, specialty appointments, follow-up care, transportation resources, and communication with internal and external healthcare facilities.
- →Manages chronic disease care and patient tracking systems by maintaining disease registries, monitoring patient follow-up, obtaining outside medical records/results, and supporting quality improvement initiatives.
- →Provides patient and family education and support through health counseling, disease prevention education, treatment guidance, and assistance in understanding medical conditions and care plans.
- →Collaborates with multidisciplinary healthcare teams including providers, nurses, specialty clinics, community resources, and support services to improve patient outcomes, safety, and healthcare delivery.
Requirements
