About the Role
Join the Indian Health Service as a Nurse Case Manager in the Purchased/Referred Care Department at the Red Lake Service Unit in Red Lake, MN. Coordinate patient referrals, assessments, care plans, authorizations, and community resources, while supporting continuity of care and positive patient outcomes. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do

- →Conducts patient assessments, develops and monitors individual care plans.
- →Coordinates care with patients, families, providers, insurers, and outside healthcare organizations.
- →Obtains authorization and connects patients with appropriate healthcare and community resources.
- →Maintains case management records, monitors patient outcomes, and prepares required reports.
- →Serves as a clinical liaison and patient advocate during care team meetings and referral activity.
- →IHS Selectees that will work in an IHS health care facility may be subject to Employee Health Policies related to Measles and Rubella, and seasonal influenza immunizations.
- →Vaccine exemption requests may be available according to the National and/or Service Unit guidance.
