What You'll Do

  • Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Manage a caseload of patients with chronic conditions by assessing needs and creating individualized care plans.
  • Coordinate care across providers, clinics and community programs to ensure patients receive timely services and care.
  • Provide education and coaching to patients and families to promote understanding, self-management, and treatment adherence.
  • Collects and maintains case management data, including service utilization, identified barriers, interventions, and patient outcomes.
  • Use patient data to identify care gaps, improve coordination processes, and support quality improvement efforts.
